• Medical Release Form - Without Insurance - To be completed by students who do not have medical insurance (in addition to Form #1).

Central Washington University (July 18-24, 2010):

• Last Minute Instructions
• CWU Recreation Center Release - If you want to use the Recreation Center ($20 charge for the week), you need to complete this form.
• CWU Climbing Release - If you want to use the Climbing Wall, you will also need to complete this form.
• College Credit Information - Please note that we cannot provide financial assistance for the fee for college credits.
• College Credit Forms (Central) - Bring this form to registration on Sunday with a check for $50. DO NOT MAIL it to Washington Business Week. Please copy the form double-sided on one piece of paper; fill in all highlighted areas.

You will need Adobe Acrobat Reader installed in order to view and print forms. Acrobat Reader is free and can be downloaded here.

If you are having trouble viewing or printing forms, you can call our office (253-815-6900) or send us an email and we will be able to mail or fax the forms to you. We also accept Visa/MasterCard over the telephone for payment of the registration fee.
